Please Note: Once you have opened the application form. Please be sure to fill it out and save the PDF to your computer. You will be able to reopen the saved application form on your computer to add and update your information. Once you have finished filling out the form, you must print the PDF out and sign the document. BCHLA requires that you mail the completed and signed form to the address specified on the Application Form.
